Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

   
Dental Insurance

You can collect your orthodontic insurance benefits even if Dr. Puccio does not participate with your insurance company. In fact, most of our patients have orthodontic benefits as part of their dental benefit package.  As a courtesy to all patients, we gladly submit all orthodontic information to your insurance company on your behalf.  Orthodontic benefits often have a lifetime maximum and are usually paid out over the months that the patient is in treatment.

The two questions to ask the insurance company directly are:

What is my lifetime orthodontic maximum benefit?  and  How exactly is it dispersed or paid out?

We can easily submit a predetermination if there is a question about what orthodontic coverage is in place.  Generally all insurance is submitted on a reimbursement basis. Your insurance benefits are paid directly to you, the subscriber, after the payment has been made to the doctor.

Dr. Puccio does not accept insurance contracts as payment in full (participate).  Insurance companies contract dentists by negotiating significantly reduced fees in exchange for sending large numbers of patients. Dr. Puccio offers high quality, low volume orthodontic care.  When choosing an orthodontist, the best value is a better gauge, not the lowest price.

Please print and fill out the Insurance form, located on our site, and bring it with you to the first appointment.  Please call if you have any insurance questions or changes in coverage.
